Chronic Care Management services are provided for at least 20 minutes by clinical staff directed by a physician or health care professional. Per calendar month

CPT 99491 Duration (30 Minutes)

Chronic care management services are provided personally by a physician or other qualified health professional at least 30 Minutes per calendar month.
